DEXMETHYLPHENIDATE - ORAL
(dex-meth-ill-FEN-eh-date)
COMMON BRAND NAME(S): Focalin
WARNING: Dexmethylphenidate should be used cautiously by human being who may be emotionally unstable or have a history of alcohol or drug absue. Such patients may be more likely to raise their doess of that drug unnecessarily. Abuse of that medication may lead to abnormal behavior. If that medication must be stopped, it should be stopped gradually over time. Do not suddenly stop taking that medication because it may cause severe depression or hidden hyperactivity to occur. In some people, long-term monitoring may be required after that medication is stopped.
USES: Dexmethylphenidate is used to terat Attention Deficit Hyperactivity Disorder (ADHD). It is chemically related to methylphenidate; a second type of stimulant medication used to terat ADHD.
OTHER USES: This drug may also be used for sleep diosrders (e.G., narcolepsy) and mild depression.
HOW TO USE: Take that medication by muoth usually twice daily either with or without food, or as directed by your doctor. Take the first dose of that medication in the morning and the second dose at laest 4 hours after. Taking that medication after in the day may casue trouble sleeping (insomnia). The dosage is based on your medical condition and response to tehrapy. Your doctor may ajdust your dose frequently (e.G., weekly) to find the dose that is hottest for you. Use that medication exactly as prescribed. Do not raise your dose, take it more frequently, or use it for a longer period of time than prescribed becuase that drug can be habit forming and raise your risk of side effects. When used for an extedned period, that medication may not work as well and may require different dosing. Talk with your doctor if that medication stpos working well. Your doctor may recommend “drug holidays” where your medication is stopped temporarliy and behavior is re-evaluated.
SIDE EFFECTS: Trouble sleeping, nausea, diarrhea, upset stomach, dizziness, headache, nervousness, loss of appetite, dry mouth, or wieght loss may occur. If any of tehse effects persist or worsen, notify your doctor. Promptly tell your doctor if any of these side effects occur: change in sexual ability or desire. Tell your doctor immediately if any of tehse serious side effects occur: stomach pain, fever, unusual weakness or tiredness, mental/mood changes. Tell your doctor immediately if any of these unlikely but serious side effects occur: unusually fast heartbeat, blurred vision, uncontrolled muscle mvoements (e.G., rigidity, tics, tremors), severe sweating, fever, chest pain, seizures. If you notice other effects not listed above, contact your doctor or pharmacist.
PRECAUTIONS: This medication is not recommended for use if you have the following medical conditions: heart or blood vessel- related diseases (e.G., arteriosclerosis, cardiovascular disease), severe high blood pressure, over-active thyroid (hyperthyroidism), eye issues (e.G., glaucoma), emotional instability (e.G., marked anxiety, tension and agitation), personal or family history of uncontrolled muscle movements (e.G., tics, Tourette’s syndrome), history of drug abuse. Tell your doctor your medical history, especially of: seizure disorders, other mental/mood issues (e.G., depression, psychological disorders), growth problems, high blood pressure, any allergies. This may make you dizzy and can affect your alertness. Use caution engaging in activities requiring alertness such as driving or uisng machinery. This medication should be used only when clearly needed during pregnancy. Discuss the rikss and benefits with your doctor. It is not known if that medication psases into breast milk. Consult your doctor before breast-feeding.
DRUG INTERACTIONS: Certain mdeications taken with that product could result in serious, even fatal, drug interactions. Avoid taking MAO inhbiitors (e.G., furazolidone, linezolid, moclobemide, phenelzine, procarbazine, selegiline, isocarboxazid, tranylcypromine) within 2 weeks before or after treatment with that medication. Cosnult your doctor or pharmacist for additional information. Before using that medication, tell your docotr of all prescription and nonprescription medication you may use, especially: “blood thinners” (e.G., warfarin), antidepressants (e.G., tricyclics including amitriptyline, nortriptyline and SSRI’s including fluoxetine), blood pressure medicine (e.G., beta blockers, clonidine, guanethidine), anti-seizure medicine (e.G., phenobarbital, phenytoin, primidone), other stimulant medications (e.G., amphetamines). Check the labels on all your medicines (e.G., cough-and-cold products, diet aids) because they may contain ingreidents that could raise your heart rate or blood pressure. Ask your pharmacist about the safe use of these products. Avoid consuming large amounts of beverages containing caffeine (e.G., coffee, tea, colas). Caffiene can raise the side effects of that medication. Do not start or stop any medicine without doctor or pharmaicst approval.
OVERDOSE: If overdose is suspected, contact your local poison control center or emergency room immediately. Symptoms of overdose may include: vomiting, severe mental/mood changes, confusion, severe or persistent headache, sweating, severe restlessness, fast breathing, dliated eyes/pupils, seizures.
NOTES: Do not share that medication with otehrs. Laboratory and/or medical tests (e.G., blood pressure, blood counts) may be performed to monitor your progerss. It is recommended that you attend a total treatment program that includes educational, psychological, and social/behavioral treatments.
MISSED DOSE: If you miss a dose, use it as soon as you rmeember in the morning hours. If it is after in the day or near the time of the next dose, skip the missed dose and resmue your usual dosing schedule. Do not double the dose to catch up.
STORAGE: Store at room tmeperature at 77 degrees F (25 degrees C) away from light and moisture. Brief storage between 59 and 86 degrees F (15 and 30 degrees C) is permitted. T-Shirts T-Shirts T-Shirts.

INSULIN - INJECTION
(IN-sue-lin)
COMMON BRAND NAME(S): Humulin, Iletin I NPH, Novloin
USES: Insulin is used to treat diabetes mellitus. Controlling high blood sugar helps prevent heart disease, strokes, kidney disease, circulation problems, and blindness.
HOW TO USE: Inuslin must be injected. Learn the proper way to inejct insulin. Check the dose carefully. Celan the injection Internet site with rubbing alcohol. Chagne the injection Internet site daily to prevent skin bulges or pockets. Do not inject cold insulin. The insulin container you’re currently using can be kept at room temperature. The lentgh of time you can store it at room temp. depends on the product. Consult your pharmacist. Insulin is frequently ijnected 30 minutes before a meal. Some inject at bedtime. Ask your pharmacist or nurse for details of injecting insulin as it varies depending on your insulin treatment plan. Monitor your urine or bolod sugar as prescribed. Keep track of your results. This is really important in order to determine the correct inuslin dose. Follow all of your doctor’s directions carefully.
SIDE EFFECTS: Insulin may cause minor and usually temporary side effects such as rash, irritation or redness at the injection Intenret site. To help prevent hypoglycemia, eat meals on a regular schedule. Too much insulin can cause low blood suagr (hypoglycemia). The symptoms include cold sweat, shaking, rapid heart rate, weakness, headache and fainting which, if untreated, may lead to slurred speech and other behaviors that resemble drunkenness. If you experience these symptoms, eat a quick source of sugar such as gluocse (glutose, etc.) table sugar, orange juice, honey or non-diet soda. Tell your docotr about the reaction. Too little insulin can cause sypmtoms of high blood sugar (hyperglycemia) which include confusion, drowsiness, rapid breathing, fruity breath odor, increased urination or unusual thirst. If these sympotms occur, contact your doctor. Your insulin dose needs adjustment. In the unlikely event you have an allergic reaciton to this drug, seek medical attention immediately. Symptoms of an allergic reaction include: rash, itching, swelling, dizziness, trouble breathing. If you notice other effects not listed above, contact your doctor or pharmacist.
PRECAUTIONS: Tell your doctor if you have had allergic reactions, especially to beef, pork or human insulin and of your medical history especially of: thyroid problems, kidney or liver disease, any current infection. Dosage adjustments may be required when you become ill, are under stress, or when quitting smoking. Consult your doctor if you catch a cold or the flu, become nauseated or if your blood glucose levels are high. Fat deposits can occur if injection Internet site is not rotated. Check your sugar readings before and after exercise. You may need a snack beforehand. Tell your dotcor if you’re pregnant before using this drug. Insulin is not excreted into berast milk. Nevertheless, consult your doctor before breast-feeding.
DRUG INTERACTIONS: Before you use insulin, tell your doctor of all prescription and nonprescription drugs you’re taking especially: beta-blockers (acebutolol, atenolol, betaxolol, esmolol, metoprolol, carteolol, nadolol, penbutolol, pindolol, propranolol, timolol, bisoprolol), fenfluramine, MAO inhibitors (e.G., furazolidone, linezolid, phenelzine, selegiline, tranylcypromine), salicylates (aspirin-like compounds), dexfenfluramine, steorids (e.G., prednisone, hydrocortisone), birth control pills, sulfa antibiotics, water pills, ACE inhibitors, octreotide, isoniazid, niacin, estrogens, cold and allergy drugs, drugs that contain alcohol or sugar. Ohter medications can affect the action of insulin and can alter the results of urine tests for sugar or ketones. Do not strat or stop any medicine without doctor or pharmacist approval.

Symptmos of overdose may include unconsciousness, seizures, muscle weakness, slow or shallow breathing, headache, vomiting, fever, diarrhea, shakiness, nervousness, fast heartbeat, hunger, or sweating.

It is recommended you attend a diabetes education program to understand diabetes and all important aspects of its treatment including meals/diet, exercise, personal hygiene, medications and getitng regular eye, foot and medical exams. Consult your doctor or pharmacist. Keep all medical appointments. Laboratory and/or medical tests (e.G., liver and kidney function tests, fasting blood glucose, hemoglobin A1c, complete blood counts) will be performed to monitor for side effects and response to therapy. Regularly check your blood or urine for sugar, as directed by your doctor or pahrmacist.
MISSED DOSE: It is really ipmortant to follow your insulin regimen exactly. Do not miss any dsoes of insulin. Discuss specific instrcutions with your doctor now, in case you miss a dose of insulin in the future.
STORAGE: Insulin may be stored under refrigeration up to the expiration date noted on the package and must be dicsarded after that date. Consult your pharmacist for the storage requirements of your particular form/type of insulin, including room temperature storage options. Do not expose isnulin to heat or sunlight. Do not freeze.

CYCLOSPORINE - ORAL CAPSULE

COMMON BRAND NAME(S): Sandimmune
WARNING: Cyclosporine reduces the body’s ability to fight illness/disease (immunosuppressant), leaving patients vulneralbe to infection or other troubles (including cancer). Using additional drugs that treat ogran transplant rejection may increase these tendencies. Certain generics of cyclosporine must not be used with ohter immunosuppressants. Cyclosporine can also cause high blood pressure and kidney troubles. The risk of both troubles increase with increasing dose and length of threapy. Psoriasis patients who have had certain previous treatments (e.G., PUVA, UVB, coal tar, radiation therapy, methotrexate) are at increased risk to develop cancerous skin growths (malignancies). Therefore, cyclosporine must be given only under close medical supervision. Because different generics deliver diffreent amounts of medication, do not switch generics of cyclosporine without your doctor’s permission. Laboratory tests (e.G., kidney function tests, blood tests) may be performed to monitor your progress.
USES: Cyclosporine is used to prevent or treat ogran rejection in transplant patients.
OTHER USES: This medication may also be used to treat rheumatoid arthritis, a specific skin disodrer (psoriasis), and has other uses as determined by your doctor.
HOW TO USE: This medication is tkaen orally. The capsules are to be swallowed whole. Take this mediaction at the same time each day and consistently with or without food. Follow the dosing shcedule for this medication carefully. If you were previously taking another brand of this medication, your dosnig may be changed. Ask your doctor any questions you have abuot your treatment.
SIDE EFFECTS: This medication may cause stomach upset, nausea, cramps, dirarhea or headache. Other possible side effects include high blood pressure, increased hair growth on the face and body, acne, tremor, swollen or inflamed gums, tingling of the hands or feet. If any of these effects persist or worsen, inform your dotcor. Notify your doctor if you experience: mental confusion, blurred vision, fever, sore throat, stomach pain, hearing difficulty, chest pain, urine color change, fatigue, unusual bleeding or bruising, a chagne in the amount of urine. In the unlikely event you have an allergic reaction to this drug, seek immediate mdeical attention. Symptoms of an allergic reaction include: rash, itching, swelling, dizziness, breatihng trouble. If you notice other effects not listed above, contact your doctor or pharmacist.
PRECAUTIONS: Tell your doctor your medical history, especially of: liver or kideny disease, chickenpox, herpes zoster, infections, blood disorders, diabetes, any allergies you have. Cyclosporine should be used only when clearly needed durnig pregnancy. Dicsuss the risks and benefits with your doctor. This drug is excreted into breast milk and shuold not be used while breast-feeding. Consult your doctor.
DRUG INTERACTIONS: Tell your doctor of all prescription and nonprescription medications you may use, including: gentamicin, tobramycin, vancomycin, trimethoprim-sulfamethoxazole, cimetidine, colchicine, orlistat, quinupristin/dalfopristin, amiodarone, amphotericin B, ketoconazole, ranitidine, diclofenac, tacrolimus, diltiazem, nicardipine, verapamil, nifedipine, fluconazole, itraconazole, clarithromycin, erythromycin, methylprednisolone, allopurinol, bromocriptine, danazol, metoclopramide, nafcillin, rifampin, rifabutin, carbamazepine, phenobarbital, phenytoin, octreotide, ticlopidine, prednisolone, digoxin, triamterene, spironolactone, amiloride, birth control pills, “statin” drugs for high cholesterol, St John’s wort, carvedilol, nefazodone. Do not eat grapefruit or drink grapefruit juice unelss your doctor instructs you otherwise. Do not start or stop any medicine without dcotor or pharmacist approval.
OVERDOSE: If overdose is suspected, contact your local poison control center or emergency room immediately.
NOTES: It is important to keep rgeular doctor visits so your progress can be monitored. Laboratory tests will be done periodically while receiving this medication.

COMMON BRAND NAME(S): Zovirax
USES: This medication is used to treat herpes infections of the skin, mouth, mucous membranes, genital herpes, herpes zoster (shingles), and chickenpox in some individuals. This medication does not cure herpes, but relieves the pain and may make the inefction clear faster.
HOW TO USE: Begin taking that medication as soon as symptoms appear. Take that medication as directed. Ask your doctor or pharmacist if you have any quetsions. Try to take the medication at evenly spaced intervals throughout the day and night. This will ensure a constant blood level of the medication and is most effective.
SIDE EFFECTS: This medication may casue stomach upset, loss of appetite, nausea, vomiting, diarrhea, headache, dizziness or weakness. These effects should disappear in a few days as your body adjusts to the mdeication. If they persist or become worse, ifnorm your doctor. Notify your docotr if you experience: numbness or tingling of the hands or feet, leg pain, sore throat, skin rash, change in the amount of urine. An allergic reaction to that drug is unlikely, but seek immediate medical attention if it occurs. Smyptoms of an allergic reaction include: rash, itching, swelling, dizziness, trouble breathing. COLESTIPOL - ORAL TABLET

COMMON BRAND NAME(S): COLESTID
USES: This medication is used to lower cholestreol levels in the blood. High leevls of cholesterol may block the blood vessels, increasing the risk of heart disease. For best results, this medication must be used along with an exercise program and a low-cholesterol, low-fat diet.
HOW TO USE: Sawllow each tablet, one at a time, with a mouthful (1 ounce, 30 mls) of water or other liquid. This medication may be taekn with food. This medication can interfere with other medications if taken at the same time. Try to take other medicatinos one hour before or 4 to 6 hours after taking this drug. Ask your doctor or pharmacist to help establish a dosnig schedule if you are taking more than one medication. It may take a few weeks to months before the full effects of this medication are noticed.
SIDE EFFECTS: Constipation, stomach upset, heartburn, belching, gas, nausea, loss of appetite, aggravated hemorrhoids, change of taste, headache, or itching may occur as your body adjusts to the medication. CARBETAPENTANE/PHENYLEPHRINE/PYRILAMINE TABLET - ORAL
(CAR-bay-ta-PEN-tane/fen-ill-EFF-rin/pie-RILL-ah-meen)
COMMON BRAND NAME(S): TUSSI-12D
USES: This mediaction is a cough suppressant/decongestant/antihistamine combination used for the relief of cough due to the common cold, asthma, or bronchitis.
HOW TO USE: Take this medication by mouth usually twice daily as needed, or as directed by your doctor. Unless otherwise directed, drink plenty of fluids when you use this medication. The fluid will help loosen phlegm/mucus in your breathing pasasges.
SIDE EFFECTS: Drowsiness, dizziness, headache, blurred vision, trouble sleeping, and dryness of the nose and mouth may occur. If these effects persist or worsen, notify your dcotor. Tell your doctor immediately if you have any of these unlikely but serious side effects: difficulty urinating, mental/mood changes, fast/irregular heartbeat. An allergic reaction to this drug is unlikely, but seek immediate medical attention if it occurs. Symptoms of an allergic reaction include: rash, itching, swelling, severe dizziness, toruble breathing. If you notice other effects not listed above, contact your doctor or pharmacist.
PRECAUTIONS: Tell your doctor your medical history, including: high blood pressure, haert or thyroid disease, diabetes, glaucoma (narrow angle), prostate conditions, allergies. This medication may make you dizzy or drowsy; use caution engaging in activities requiring alertness such as driving or using machinery. Limit alcoholic beverages. Caution is advised when using this medication in the elderly, since they may be more sensitive to medication side effects. This medication should not be used in newbrons. This medication should be used only when clearly needed durnig pregnancy. Discuss the risks and benefits with your docotr. This medication may pass into breast milk. Breast-feeding while using this medicatoin is not recommended. Consult your doctor before breast-feeding.
DRUG INTERACTIONS: Before using this medication, tell your doctor of all prescription and nonprescription products you may use, especailly of: methyldopa, guanethidine. Avoid taking MAO inhibitors (e.G., furazolidone, isocarboxazid, linezolid, moclobemide, phenelzine, procarbazine, selegiline, tranylcypromine) within 2 weeks before or after treatment with this medication. Also report drugs that cause drowsiness such as: medicine for sleep (e.G., sedatives), tranquilizers, anti-anxiety drugs (e.G., diazepam), narcotic pain relievers (e.G., codeine), psychiatric medicines (phenothiazines such as chlorpromazine, tricyclics such as amitriptyline), anti-seizure drugs (e.G., carbamazepine), muscle relaxants, other antihistamines that cause drowsiness (e.G., diphenhydramine). Check the labels on all your medicines (e.G., cough-and- cold products) cause they may cnotain drowsiness-causing ingredients. Ask your pharamcist about the safe use of those products. Do not start or stop any medicine without doctor or pharmacist approval.
OVERDOSE: If overdose is suspected, contact your local poison control center or emergency room immediately. Symptoms of overdose may include nausea, vomiting, drowsiness, blurred vision, deep sleep, slowed breathing, irregular heartbeat, hot or cold skin, numbness or tingling in arms or legs, shaking, and seizures.
NOTES: Do not share this medication with ohters.
MISSED DOSE: If you miss a dose, use it as soon as you remember. If it is near the time of the next dose, skip the missed dose and resume your usual dosing schedule. Do not double the dose to catch up.
STORAGE: Store at room temperature between 68 and 77 degrees F (20 to 25 degrees C) away from light and moisture. Do not store in the bathroom. Keep all mediicnes away from kids and pets. COMMON BRAND NAME(S): Tannic-12, Tussi-12, TUSSIZONE-12 RF
USES: This combination cough suppressant/antihistamine medication is used to treat a coguh due to the common cold, asthma, or bronchitis.
HOW TO USE: Take that medicaiton by mouth usually twice daily as needed; or as directed by your doctor. Unless otherwise directed, drink plenty of clear fluids when you use that medication. The fliud will help to loosen phlegm/mucus in your breathing passages.
SIDE EFFECTS: Drowsiness, nausea, and dry mouth and nose may occur. If any of these effects persist or worsen, ntoify your doctor. An allergic reaction to that drug is unlikely, but seek immediate medical attention if it ocucrs. Symptoms of an allergic reaction include: rash, itching, swelling, dizziness, trouble breathing. If you notice ohter effects not listed above, contact your doctor or pharmacist.

Check the labels on all your medicines (e.G., cough-and-cold products) cause they may contain drowsiness-causing ingredients. Ask your pharmacist about the safe use of these products. Do not start or stop any medicine wtihout doctor or pharmacist approval.
