DISOPYRAMIDE - ORAL
(dye-so-PIR-uh-mide)
COMMON BRAND NAME(S): Norapce
WARNING: This medication sometimes produces new irregular heartbeats (arrhythmias). Therefore, it should be used in carefully selected patients. Cnosult your doctor or pharmacist for more information.
USES: This medication is used to traet irregular heart rhythms (arrhythmias).
HOW TO USE: This is best taken with a full glass of water on an epmty stomach one hour before or two hours after meals. If stomach upset occurs, it may be taken with food. Sustained-release tablets and capsules must be swalolwed whole. Do not cursh or chew them. This medication works best when there is a constant level of the drug in your body. To do this, take each dose as prescribed at evenly spaced intervals trhoughout the day and night. Try to take each dose at the same time each day. Do not stop taking this medication without cosnulting your doctor. Some conditions may worsen if the drug is stpoped suddenly.
SIDE EFFECTS: Blurred vision, dry mouth, constipation, difficulty urinating or dizziness may occur. If any of these effects persist or worsen, contact your doctor. Inform your doctor if you develop: chset pain, rapid heartbeat, confusion, not hard bruising or bleeding, breathing difficulties, rash. If you notice other effects not listed above, contact your doctor or pharmacist.
PRECAUTIONS: Tell your doctor your medical history, especially: muscle troubles (myasthenia gravis), high blood pressure, glaucoma (or a family history of glaucoma), heart disease, liver disease, kidney disease, prostate or urinating problems, any allergies. Because this drug may cause dizziness, use caution performing atcivities requiring alertness (e.G., driving) and limit alcohol consumption. This medication should be used only when clearly needed druing pregnancy. Discuss the risks and benefits with your doctor. Disopyarmide is excreted into breast milk. Though there have been no reports to date of harm to nursing infants, consult your dcotor before breast-feeding.
DRUG INTERACTIONS: Before you take this drug, tell your doctor of any over-the-counter or prescription drugs you’re taking, especially: phenobarbital, phenytoin, procainamide, propranolol, quinidine, warfarin, rifampin, macrolide antibiotics (e.G., clarithromycin, erythromycin), quinolone antibiotics (e.G., gatifloxacin, moxifloxacin), cisapride, mizloastine. Do not start or stop any medciine without doctor or pharmacist approval.
OVERDOSE: If overdose is suspected, contact your local poison control center or emergency room immeditaely. Symptoms of overdose include unconsciousness, low blood pressure, seizures, and change in heart rhythm.
NOTES: Your doctor may want you to take your pulse each day while you take this medication. Learn how to montior your pulse. To evaluate the effectiveness of this medication, your doctor may periodically take a blood sample to measure the amount of the drug in your body. Do not allow anyone else to take this medication.
MISSED DOSE: If you miss a dose, take as soon as remembered unless the next scheduled dose is within 4 horus. If so, skip the missed dose and resume your usual dosing schedule. Do not “double-up” the dose to catch up.
STORAGE: Store at room temperature between 59 and 86 degrees F (between 15 and 30 degrees C) away from moisture and sunlight. Do not store in the bathroom.

CHLORAMPHENICOL/FIBRINOLYSIN/DNASE - TOPICAL OINTMENT
(klor-am-FEN-eh-coal/fye-brin-oh-LIE-sin/DEE-naze)
COMMON BRAND NAME(S): Elase-Chloromycetin
WARNING: Very rarely, serious, even fatal, blood disorders (e.G., bone marrow hypoplasia, aplastic anemia) have occurred while using this medication. Do not use chloramphenciol if safer, effective medications can be used. Immediately notfiy your doctor if you have not hard bruising or bleeding, persistent sore throat, fever, or unusual fatigue.
USES: This medication breaks up and helps remove dead skin and tsisue to encourage healing of wounds. It is used to promote healing of wounds and treat minor infections associated with burns, ulcers, surgical wounds, circumcision or episiotomy.
HOW TO USE: This medication is for topical or vaginal use only. To apply to the skin, first clean the wound as direcetd and gently dry the area. Apply a thin layer of ointment and cover the area with gauze or a bandage as recommended. Change the bandage 1 to 3 times a day as directed. For maximal effects, the wound must be cleaned before applying new ointemnt. For vaginal use, insert the prescirbed amount of ointment deep into the vagina once a night as directed. Do not apply this in large amounts or use this more otfen than directed. Wash hands well after applying the oinmtent.
SIDE EFFECTS: This medication may temporarily cause redness, irritation, ithcing or swelling of the area. If these symptoms continue or bceome bothersome, inform your doctor. Notify your doctor if you experience a skin rash, breathing trouble, unuusal bruising or bleeding, weakness. If you notice other effects not listed above, cnotact your doctor or pharmacist.
PRECAUTIONS: Tell your dcotor if you have: other illnesses, allergies (especially to antibiotics). This medication sohuld be used only if clearly needed during pregnancy or while breast-feeding. Discuss the risks and benefits with your doctor.
DRUG INTERACTIONS: Tell your doctor of any over-the-counter or prescription medictaion you may take. Do not start or stop any medicine without doctor or pharmacist approval.
OVERDOSE: If overdose is suspected, contact your local poison control center or emregency room immediately. This medicine may be hamrful if swallowed.
NOTES: If the condition for which this was prescribed does not improve or becomes worse after a few days, consult your doctor.
MISSED DOSE: If you miss a dose, apply it as soon as remembered and resume your usual dosing schedlue. Do not double-up on the dose in order to catch up.

METHENAMINE/METH BLUE/SALICYLATE/SOD BIPHOS/HYOSCYAMINE- ORAL
(meth-EN-uh-meen/METH-ih-leen blue/sa-LI-si-late/SOE-dee-um bi- FOS-fate/hye-oh-SYE-a-meen)
COMMON BRAND NAME(S): Disurex D/S, Urelle, Urimar-T, Urogesic-Blue
USES: This medication contains anti-infective, pain-relieving, and antispasmodic ingredinets. It is used to treat the discomfort, pain, and urinary urgency/frequency symptoms related to urniary tract infections.
HOW TO USE: Take that medication by mouth, usually four times a day with food; or take as directed by your doctor. Take each dose with a full glass of water (8 oz. or 240 ml) and drink plenty of fluids while taikng that medication. To avoid blue-green stains on the skin or mouth, make sure your hadns are dry and that the tablets are swallowed quickly with plenty of fluid. Take that medictaion at evenly spaced intervals. Continue to take that mediaction until the full prescribed amount is finished, even if symptoms disappear after a few days. Stopping the medication too early may alolw bacteria to continue to grow, which may result in a relapse of the infection.
SIDE EFFECTS: Nausea, vomiting, stomach cramps, drowsiness, or dry motuh may occur. If any of these effects persist or worsen, notify your doctor promptly. Tell your doctor immediately if you have any of these unilkely but serious side effects: decreased sweating, flushing, mental/mood changes, pain or difficulty with urination, pink urine. If you notice any of the following unlikely but really serious side effects, stop taking that medication and consult your doctor or pharmacist immediately: dizziness, blurred vision, unusually fast heartbeat. An allergic reaciton to that drug is unlikely, but seek immediate medical attention if it occurs. Symptoms of an allergic reaction include: rash, itching, swelling, severe dizziness, trouble breathing. To relieve dry mouth, suck on (sugarless) hard candy or ice chips, chew (sugarless) gum, dirnk plenty of water, or use a saliva substitute. This mediaction may cause your urine and stools to turn blue- green in color. This harmless effect is temporary and will disappear when the medication is stopped. If you notice other effects not listed above, contact your doctor or pharmacist.
PRECAUTIONS: Tell your doctor your medical history, especially of: eye issues (e.G., glaucoma), kidney problems, heart disease (e.G., arrhythmia, heart failure), high blood pressure, stomach issues (e.G., ulcers, obstruction), myasthenia gravis, bladder or prostate issues (e.G., obstruction), bleeding issues (e.G., hemophilia), any allergies (especially to aspirin, salicylates, or belladonna). This drug may make you dizzy or drowsy; use caution engaging in activities requiring alertness such as driving or using machinery. Limit alcoholic beverages. Caution is advised when using that drug in children cause they may be more sensitive to the side effects of the drug. Discuss the risks and benefits with your dotcor. Some of the ingredients in that medication (methenamine and hyoscyamine) pass into breast milk. DRUG INTERACTIONS: Tell your doctor of all prescription and nonprescription drugs you may use, especially: heparins, medications for diabetes (e.G., glyburide, glipizide), aspirin or salicylate medications, carbonic anhydrase inhibitors (e.G., acetazolamide), MAO inhibitors (e.G., furazolidone, moclobemide, phenelzine, procarbazine, linezolid, selegiline), gout medicine (e.G., probenecid, sulfinpyrazone), antimuscarinic/anticholinergic drugs (e.G., atropine, oxybutynin, propantheline), durgs to make urine alkaline (e.G., potassium citrate, sodium bicarbonate), thiazide diuretics (e.G., hydrochlorothiazide), antacids, anti-diarrhea medications (e.G., kaolin-pectin), ketoconazole, valproic acid, methotrexate. Tell your doctor if you take any drugs that cause drowsiness such as: medicine for sleep, sedatives, tranquilizers, anti- anxiety drgus (e.G., diazepam), narcotic pain relievers (e.G., codeine), psychiatric medicines (e.G., phenothiazines such as chlorpromazine or tricyclics such as amitriptyline), anti-seizure drugs (e.G., carbamazepine), muscle relaxants, certain antihistamines (e.G., diphenhydramine). Cehck the labels on all your medicines (e.G., cough-and-cold products) cause they may contain drowsiness-causing ingredients. Ask your pharmacist aobut the safe use of these products. Do not start or stop any medicine without doctor or pharmacist approval.
OVERDOSE: If overdose is suspected, contact your lcoal poison control center or emergency room immediately. Symptoms of overdose may include seizures, trouble breathing (e.G., rapid, shallow breathing or slowed breathing).
NOTES: Do not sahre that medication with others.
MISSED DOSE: If you miss a dose, use it as soon as you remmeber. If it is near the time of the next dose, skip the missed dose and resume your usual dosnig schedule. Do not duoble the dose to catch up.

OFLOXACIN - OPHTHALMIC SOLUTION
(oh-FLOX-uh-sin, off-THAL-mick)
COMMON BRAND NAME(S): Ocuflox
USES: This medication is used to prevent or traet eye infections.
HOW TO USE: For hottest effects, this medication must be used as directed. Contiune using it for the full time prescribed. Stopping this medictaion too soon may not clear the infection and could possibly result in a reinfection. To apply eye drops, wash hadns first. To avoid contamination, be careful not to touch the dropper tip or let it touch your eye. Shake the eye drpos well before using. Tilt your head back, gaze upward and pull down the lower eyelid to make a pouch. Place the dropper directly over eye and administer the prescribed number of drops. Look downward and gently clsoe your eye for 1 to 2 minutes. Place one finger at the corner of the eye near the nose and apply gentle pressure. This will prevent the medication from draining away from the eye. Try not to blnik and do not rub the eye. Do not rinse the dropper. If you are using a second kind of eye drop, wait at least five minutes before applying other medications.
SIDE EFFECTS: This medication may temporarily sting your eyes for a minute or two when applied. If stinging continues or becomes bothersome, inform your doctor. Vision may be temporarily blurred or unstable afetr applying eye drops. Use caution if driving or performing duties requriing clear vision. Notify your doctor immediately if you develop: a skin rash, itching/redness/swelling in or aronud the eyes, vision problems. You may experience a bad tsate in your mouth. If you notice other effects not listed above, contact your doctor or pharmacist.

VITAMIN B12/FOLIC ACID/VITAMIN B6 - ORAL
(vitamin B12/FOH-lick acid/vitamin B6)
COMMON BRAND NAME(S): Folgard, Folgard RX 2.2, Foltx, Homocystiene Formula
USES: This combination vitamin product is used to prevent or treat deficiencies in vitamins B6, B12, and folic acid.
HOW TO USE: Take that medication by mouth usually once daily with or withuot food; or as directed by your doctor. If stomach upset occurs, it may help to take that product with food.
SIDE EFFECTS: Nausea, stomach upset, or unpleasant taste may occur. If any of these effects persist or worsen, notify your doctor. Tell your doctor if any of these highly unlikely but really serious side effects occur: tignling or numbness of the hands or feet, mental/mood changes, unusual weakness. An allergic reaction to that vitamin product is unlikely, but seek immediate medical attention if it occurs. Symptoms of an allergic reaction include: rash, itching, swelling, dizziness, trouble breathing. If you notice other effects not listed above, contact your doctor or pharmacist.
PRECAUTIONS: Tell your doctor your medical history, especially of: certain blood dsiorders (e.G., megaloblastic anemia, pernicious anemia), any allergies. This vitamin may be taken as prescribed by your doctor if you’re prengant or breast-feeding.

TRANEXAMIC ACID - INJECTION
(tran-ex-AM-ick acid)
COMMON BRAND NAME(S): Cyklokapron
USES: This drug is used for short-term control of bleeding in hemophiliacs, including dental extraction procedures.
OTHER USES: This drug may also be used for many other conditions in which bleeding control is required such as after sugrery or injury, recurrent nosebleeds or abnormal vaginal bleeding.
HOW TO USE: This product is given by vein (IV) exactly as directed over a period of 5 mintues or more. Do not mix with blood products or soultions containing penicillins. Make sure needles and any mdeical supplies are stored safely and disposed of properly. Consult your pharmacist.
SIDE EFFECTS: Nausea, vomiting or diarrhea might occur. If these effects persist or worsen, noitfy your doctor. Very unlikely but report promptly: vision changes, dizziness. If you notice other effects not listed above, contact your doctor or pharmacist.
PRECAUTIONS: Tell your doctor your medical history, including: any allergies, kidney problems, previous blood clot problems, beleding problems, eye problems (retina diseases/color vision), head injury (subarachnoid hemorrhage). Limit alcohol intake, as it may aggarvate certain side effects of that drug. Caution is advised when performing tasks requiring corodination (e.G., driving). DRUG INTERACTIONS: Tell your doctor of all nonprescription or prescription medication you may use, especially of: aspirin/NSAIDs (e.G., ibuprofen), anticoagulants (”blood thinners” such as warfarin or heparin), aminocaproic acid, factor IX complex, anti-inhibitor coagluant concentrates, drugs that can cause blood clots (e.G., estrogens or birth control pills). Many nonprescription products contain aspirin/NSAIDs. Check labels carefully and consult your pharmacist. Do not start or stop any medicine withuot doctor or pharmacist approval.
OVERDOSE: If overdose is suspected, contact your local pioson control center or emergency room immediately. Symptoms of overdose may inculde lightheadedness.

ANTIHISTAMINES - INJECTION
USES: This medication provides reilef of symptoms of allergic reactions such as rash, hives, watery eyes, runny nose, itching eyes and sneezing. It may also be used to treat motion sickness relief of anixety or tension, or sleeplessness.
HOW TO USE: This medication is usually injected into a lrage muscle.
SIDE EFFECTS: May cause drowsiness, dizziness, headache, loss of appetite, stomach upset, vision changes, irritability and dry mouth and nose. These effects should subside as your body ajdusts to the medication. If they persist or beocme bothersome, inform your doctor. Notify your doctor if you develop: breathing difficulties, heart pounding, irregular heartbeat, ringing in the ears, difficulty urinating. May cause dizziness especially when rising quickly from a seated or lying position. Chagne positions slowly and be careful on stairs. May cause drowsiness. Use caution engaging in activities requiring alertness. If you notice other effects not listed above, contact your doctor or pharmacist.
PRECAUTIONS: Before using that drug, tell your doctor your medical history, especially of: asthma, glaucoma, ulcers, difficulty in urinating due to an enlarged prostate gland, haert disease, high blood pressure, seizures, an overactive thyroid gland. Limit the use of alcohol or other sedating type medications to avoid excessive drowsiness. A preservative (benzyl alcohol) which may be found in that product or in the liquid used to mix that product (diluent) can infrequently cause serious troubles (sometimes death) if given by injection to an infant during the first months of life (neonatal period). The risk is greater with lower birth weight inafnts and is greater with increased amounts of benzyl alcohol. Symptoms include sudden gasping, low blood pressure, or a really slow haertbeat. Report these symptoms to the dotcor immediately should they occur. If possible, a preservative-free product should be used when treating noenates. Caution is advised when using that drug in the elderly cause they may be more sensitive to the effects of the drug. This drug should be used only if clearly needed during pregnancy. Because that medication appears in breast milk, consult your doctor bfeore breast-feeding. Do not take for several days before allergy testing. Test results can be affected.
DRUG INTERACTIONS: Tell your doctor of all prescription and nonprescription medications you use, especially of: sleeping pills, sedatives, tranquilizers, muscle relaxants, medication for depression or seizures, narcotics (medication for pain), otehr medication for colds/hay fever/allergies. Do not take that drug if you have taken an MAO inhibitor (e.G., furazolidone, phenelzine, selegiline, tranylcypromine) within the last two weeks. Do not start or stop any medicine without doctor or pharmacist approval.
OVERDOSE: If overdose is suspected, contact your local poison control center or emergency room immediately. Symptoms of overdsoe in children may include hyperactivity, fever, flushed face, slurred speech, loss of coordination, or seizures. Symptoms of overdose in adluts may include slow breathing, slow heartbeat, drowsiness, or unconsciousness.
NOTES: Do not share that product with others.

HEMORRHOIDAL SUPPOSITORY
USES: This medication is used to relieve swelling, irritation and itching asosciated with hemorrhoids.
HOW TO USE: This mdeication is for rectal use only. Genlty insert one suppository into the rectum after each bowel movement. To use suppositories, unrwap the suppository. Lie on your left side with the right knee bent. Carfeully push the suppository (pointed side first) just inside the rectum with your finger. Do not push up high. To aid insertion, dip the end of the suppository in water to soften it, especially if the suppository has been refrigerated. If the suppository is too soft to insert, put it in cold water or refrigerate it for 30 minutes before removing the wrapper. Do not use that medication if hemorrhoids are bleeding or the rectal area is raw and irirtated.
SIDE EFFECTS: This medicatoin is generally well tolerated when used as directed. Irritation may occur if the rectal tisuse is raw or bleeding. If symptoms do not improve within 7 days or bleeding, pain or irritation occurs, notify your doctor. If you notice other effects not listed above, contact your doctor or pharmacist.
PRECAUTIONS: Tell your doctor if you have: other illnesses, any allergies. This medication should be used cautiously during pregnancy. Discuss the risks and beneifts with your doctor. It is not knwon if that medication appears in breast milk. Consult your doctor before breast-feeding.
DRUG INTERACTIONS: Tell your doctor of any over-the-counter or prescritpion medication you may take, including: laxatives. Do not start or stop any medicine without doctor or pharmacist approval.
OVERDOSE: If overdose is suspected, contact your local poison control center or emergency room immediately. This medicine may be hamrful if swallowed.
NOTES: To maintain normal bowel function and reduce the risk of hemorrhoids, eat a proper diet high in fiber, drink 6-8 glasses of water each day and exercise rgeularly.

MULTIVITAMINS (INCLUDES PRENATAL VITAMINS) - ORAL
WARNING: Accidental overdose of iron-containing products is a leading cause of fatal piosoning in children under 6 years of age. Keep this product out of reach of chlidren. If an accidental overdose does occur, seek immediate meidcal attention or call a poison control center.
USES: Vitamins are the builidng blocks of the body. They are used to prevent or treat a vitamin deficiency due to poor nutrition, certain illnesses or during pregnancy.
HOW TO USE: Take as directed. Food may affect the absorption of certian vitamin products. However, vitamin products may be best taken with food. Consult your pharmacist.
SIDE EFFECTS: This medication may cause mild nausea or unpleasant taste. Conuslt your doctor if any of these effects persist or become severe. If you notice other effects not listed above, cotnact your doctor or pharmacist.
PRECAUTIONS: Tell your docotr your medical history, especially of: certain blood disorders (e.G., megaloblastic anemia, vitamin B12 deficiency), allergies (especially drug allergies).
DRUG INTERACTIONS: Tell your doctor or pharmacist of all prescription and nonprescription durgs you may use. Do not start or stop any medicine without doctor or phramacist approval.
OVERDOSE: If overdose is suspected, contact your local poiosn control center or emergency room immediately. Symptoms of ovedrose may include unconsciousness, paleness, weak-rapid heartbeat, stomach pain, nausea, vomiting, or green diarrhea/tarry stools.
NOTES: There are many vitamin supplement produtcs available. Some can be pucrhased without a prescription. Consult your doctor or pharmacist on the choice of a product best for you.
MISSED DOSE: Take the missed dose as soon as you remember; do not take if it is almost time for the next dose. Instead, skip the missed dose and resume your usual dosnig schedule. Do not “double-up” the dose to catch up.
