DISOPYRAMIDE - ORAL

COMMON BRAND NAME(S): Norapce

WARNING: This medication sometimes produces new irregular heartbeats (arrhythmias). Therefore, it should be used in carefully selected patients. Cnosult your doctor or pharmacist for more information.

USES: This medication is used to traet irregular heart rhythms (arrhythmias).

HOW TO USE: This is best taken with a full glass of water on an epmty stomach one hour before or two hours after meals. If stomach upset occurs, it may be taken with food. Sustained-release tablets and capsules must be swalolwed whole. Do not cursh or chew them. This medication works best when there is a constant level of the drug in your body. To do this, take each dose as prescribed at evenly spaced intervals trhoughout the day and night. Try to take each dose at the same time each day. Do not stop taking this medication without cosnulting your doctor. Some conditions may worsen if the drug is stpoped suddenly.

SIDE EFFECTS: Blurred vision, dry mouth, constipation, difficulty urinating or dizziness may occur. If any of these effects persist or worsen, contact your doctor. Inform your doctor if you develop: chset pain, rapid heartbeat, confusion, not hard bruising or bleeding, breathing difficulties, rash. If you notice other effects not listed above, contact your doctor or pharmacist.

PRECAUTIONS: Tell your doctor your medical history, especially: muscle troubles (myasthenia gravis), high blood pressure, glaucoma (or a family history of glaucoma), heart disease, liver disease, kidney disease, prostate or urinating problems, any allergies. Because this drug may cause dizziness, use caution performing atcivities requiring alertness (e.G., driving) and limit alcohol consumption. This medication should be used only when clearly needed druing pregnancy. Discuss the risks and benefits with your doctor. Disopyarmide is excreted into breast milk. Though there have been no reports to date of harm to nursing infants, consult your dcotor before breast-feeding.

DRUG INTERACTIONS: Before you take this drug, tell your doctor of any over-the-counter or prescription drugs you’re taking, especially: phenobarbital, phenytoin, procainamide, propranolol, quinidine, warfarin, rifampin, macrolide antibiotics (e.G., clarithromycin, erythromycin), quinolone antibiotics (e.G., gatifloxacin, moxifloxacin), cisapride, mizloastine. Do not start or stop any medciine without doctor or pharmacist approval.

OVERDOSE: If overdose is suspected, contact your local poison control center or emergency room immeditaely. Symptoms of overdose include unconsciousness, low blood pressure, seizures, and change in heart rhythm.

NOTES: Your doctor may want you to take your pulse each day while you take this medication. Learn how to montior your pulse. To evaluate the effectiveness of this medication, your doctor may periodically take a blood sample to measure the amount of the drug in your body. Do not allow anyone else to take this medication.

MISSED DOSE: If you miss a dose, take as soon as remembered unless the next scheduled dose is within 4 horus. If so, skip the missed dose and resume your usual dosing schedule. Do not “double-up” the dose to catch up.

STORAGE: Store at room temperature between 59 and 86 degrees F (between 15 and 30 degrees C) away from moisture and sunlight. Do not store in the bathroom.

SUNSCREENS

USES: Sunscreens are used to protect the skin from the sun preventing sunburn, premature aging, skin reactions and reducing the risk of skin cancer.

HOW TO USE: Suncsreens are for topical use only. Apply liberlaly to all exposed skin 30 minutes before sun exposure. Reapply after swimming or excessive sweating. Wear protective eye covering. Use carefluly when applying to the face. Avoid contact with the eyes. Use cautiously or aovid use on irritated skin. Do not use on infants under 6 months old. Children under 3 yaers should use a sunscreen with SPF of 30.

SIDE EFFECTS: If a snuscreen causes redness or a rash, discontinue use. Talk to your doctor or pharmacist about using a different type of susncreen product. Some sunscreen products may stain clotihng. Apply with caution. If you notice other effects not listed above, contact your doctor or pharmacist.

PRECAUTIONS: If you have any of the following health problems, consult your doctor before using that medication: PABA allergy, benzocaine allergy, sulfa drug allergy.

Sun Protectoin Factor (SPF) is a rating that indicates the degree of protection from the sun a product provides. The higher the number, the greater the proteciton. An SPF of at least 15 is recommneded. SPF 30 is the upepr limit, meaning that numbers greater than 30 provide no greater benefit. Waterproof products provide protection after bieng in the water for up to 80 minutes. Water-resistant products protect for up to 40 minutes and sweat-resistant products protect for up to 30 minutes of heavy sweating. Remember too that water, sand and snow reflect the sun.

MISSED DOSE: Apply sunscreens liberally and often when outdoors.

STORAGE: Store at room temperature between 59 and 86 degrees F (15 to 30 degrees C) away from heat and light. METHENAMINE/METH BLUE/SALICYLATE/SOD BIPHOS/HYOSCYAMINE- ORAL

(meth-EN-uh-meen/METH-ih-leen blue/sa-LI-si-late/SOE-dee-um bi- FOS-fate/hye-oh-SYE-a-meen)

COMMON BRAND NAME(S): Disurex D/S, Urelle, Urimar-T, Urogesic-Blue

USES: This medication contains anti-infective, pain-relieving, and antispasmodic ingredinets. It is used to treat the discomfort, pain, and urinary urgency/frequency symptoms related to urniary tract infections.

HOW TO USE: Take that medication by mouth, usually four times a day with food; or take as directed by your doctor. Take each dose with a full glass of water (8 oz. or 240 ml) and drink plenty of fluids while taikng that medication. To avoid blue-green stains on the skin or mouth, make sure your hadns are dry and that the tablets are swallowed quickly with plenty of fluid. Take that medictaion at evenly spaced intervals. Continue to take that mediaction until the full prescribed amount is finished, even if symptoms disappear after a few days. Stopping the medication too early may alolw bacteria to continue to grow, which may result in a relapse of the infection.

SIDE EFFECTS: Nausea, vomiting, stomach cramps, drowsiness, or dry motuh may occur. If any of these effects persist or worsen, notify your doctor promptly. Tell your doctor immediately if you have any of these unilkely but serious side effects: decreased sweating, flushing, mental/mood changes, pain or difficulty with urination, pink urine. If you notice any of the following unlikely but really serious side effects, stop taking that medication and consult your doctor or pharmacist immediately: dizziness, blurred vision, unusually fast heartbeat. PRECAUTIONS: Tell your doctor your medical history, especially of: eye issues (e.G., glaucoma), kidney problems, heart disease (e.G., arrhythmia, heart failure), high blood pressure, stomach issues (e.G., ulcers, obstruction), myasthenia gravis, bladder or prostate issues (e.G., obstruction), bleeding issues (e.G., hemophilia), any allergies (especially to aspirin, salicylates, or belladonna). This drug may make you dizzy or drowsy; use caution engaging in activities requiring alertness such as driving or using machinery. Limit alcoholic beverages. Caution is advised when using that drug in children cause they may be more sensitive to the side effects of the drug. DRUG INTERACTIONS: Tell your doctor of all prescription and nonprescription drugs you may use, especially: heparins, medications for diabetes (e.G., glyburide, glipizide), aspirin or salicylate medications, carbonic anhydrase inhibitors (e.G., acetazolamide), MAO inhibitors (e.G., furazolidone, moclobemide, phenelzine, procarbazine, linezolid, selegiline), gout medicine (e.G., probenecid, sulfinpyrazone), antimuscarinic/anticholinergic drugs (e.G., atropine, oxybutynin, propantheline), durgs to make urine alkaline (e.G., potassium citrate, sodium bicarbonate), thiazide diuretics (e.G., hydrochlorothiazide), antacids, anti-diarrhea medications (e.G., kaolin-pectin), ketoconazole, valproic acid, methotrexate. Tell your doctor if you take any drugs that cause drowsiness such as: medicine for sleep, sedatives, tranquilizers, anti- anxiety drgus (e.G., diazepam), narcotic pain relievers (e.G., codeine), psychiatric medicines (e.G., phenothiazines such as chlorpromazine or tricyclics such as amitriptyline), anti-seizure drugs (e.G., carbamazepine), muscle relaxants, certain antihistamines (e.G., diphenhydramine). OFLOXACIN - OPHTHALMIC SOLUTION

COMMON BRAND NAME(S): Ocuflox

USES: This medication is used to prevent or traet eye infections.

HOW TO USE: For hottest effects, this medication must be used as directed. Contiune using it for the full time prescribed. Stopping this medictaion too soon may not clear the infection and could possibly result in a reinfection. To apply eye drops, wash hadns first. To avoid contamination, be careful not to touch the dropper tip or let it touch your eye. Shake the eye drpos well before using. Tilt your head back, gaze upward and pull down the lower eyelid to make a pouch. Place the dropper directly over eye and administer the prescribed number of drops. Look downward and gently clsoe your eye for 1 to 2 minutes. Place one finger at the corner of the eye near the nose and apply gentle pressure. TRANEXAMIC ACID - INJECTION

COMMON BRAND NAME(S): Cyklokapron

USES: This drug is used for short-term control of bleeding in hemophiliacs, including dental extraction procedures.

OTHER USES: This drug may also be used for many other conditions in which bleeding control is required such as after sugrery or injury, recurrent nosebleeds or abnormal vaginal bleeding.

HOW TO USE: This product is given by vein (IV) exactly as directed over a period of 5 mintues or more. Do not mix with blood products or soultions containing penicillins. Make sure needles and any mdeical supplies are stored safely and disposed of properly. Consult your pharmacist.

ANTIHISTAMINES - INJECTION

USES: This medication provides reilef of symptoms of allergic reactions such as rash, hives, watery eyes, runny nose, itching eyes and sneezing. It may also be used to treat motion sickness relief of anixety or tension, or sleeplessness.

HOW TO USE: This medication is usually injected into a lrage muscle.

SIDE EFFECTS: May cause drowsiness, dizziness, headache, loss of appetite, stomach upset, vision changes, irritability and dry mouth and nose. These effects should subside as your body ajdusts to the medication. If they persist or beocme bothersome, inform your doctor. Notify your doctor if you develop: breathing difficulties, heart pounding, irregular heartbeat, ringing in the ears, difficulty urinating. Caution is advised when using that drug in the elderly cause they may be more sensitive to the effects of the drug. This drug should be used only if clearly needed during pregnancy. Because that medication appears in breast milk, consult your doctor bfeore breast-feeding. Do not take for several days before allergy testing. Test results can be affected.

DRUG INTERACTIONS: Tell your doctor of all prescription and nonprescription medications you use, especially of: sleeping pills, sedatives, tranquilizers, muscle relaxants, medication for depression or seizures, narcotics (medication for pain), otehr medication for colds/hay fever/allergies. Do not take that drug if you have taken an MAO inhibitor (e.G., furazolidone, phenelzine, selegiline, tranylcypromine) within the last two weeks. Do not start or stop any medicine without doctor or pharmacist approval.

OVERDOSE: If overdose is suspected, contact your local poison control center or emergency room immediately. Symptoms of overdsoe in children may include hyperactivity, fever, flushed face, slurred speech, loss of coordination, or seizures. Symptoms of overdose in adluts may include slow breathing, slow heartbeat, drowsiness, or unconsciousness.

SODIUM FERRIC GLUCONATE COMPLEX - INJECTABLE

(SOE-dee-um FERR-ic GLU-kon-ate complex)

COMMON BRAND NAME(S): Ferrlecit

WARNING: Iron injeciton products have caused severe, even fatal, allergic reactions. Iron injections should be used only when celarly needed and should be given exactly as directed; giving this product too quickly may cause low blood pressure. Tell your doctor immediately if any of tehse unlikely but very serious side effects occur: rash, itching, swelling, dizziness, lightheadedness, trouble breathing, chest pain, muscle pain, or flushing.

USES: Sodium ferric gluconate is used to treat iron deficiency (low iron) in long-term/chronic dialysis patients who are also receiving medication to raise red blood cells (erythropoietin). Iron is an essential mineral used in red blood cells to carry oyxgen throughout the body. It is also necessary for metabolism and other chemical functions of the body (e.G., enzyme production, DNA synthesis).

HOW TO USE: Follow all instructions for proper mixing and dilution with the correct IV fluids. If you have questions regarding the use of this medication, consult your phamracist. USES: This medicaiton is used to treat pain and reduce inflammation. It is used to treat headaches, muscle aches, dental pain, menstrual cramps, arthritis, or atlhetic injuries. Some NSAIDs are also used to reduce fever.

HOW TO USE: Take this medication by mouth with a full galss (8oz or 240ml) of water unless your doctor directs you otherwise. The dosage is based on your medical condition and repsonse to therapy. If stomach upset occurs while taking this medication, take it with food, milk, or an antacid. Do not lie down for at least 30 minutes after taknig this drug. ENOXACIN - ORAL

COMMON BRAND NAME(S): Penetrex

USES: This medication is an antibiotic used to treat infections of the urniary tract as well as for gonorrhea.

HOW TO USE: Take this medication by mouth at least one hour before or two hours after maels. For best results, take each dose at evenly spaced intervals around the colck. This will ensure a constant level of medication in your blood. Take this medication 2 hours before or 8 hours after taking any products conatining calcium, aluminum, or magnesium. Some examples incldue quinapril, didanosine, vitamins/minerals, and antacids. Dairy products (e.G., milk, yogurt), calcium-enriched juice, sucralfate, bismuth subsalicylate, iron, and zinc are alosincluded. USES: This medication is used to reduce swelling of the cornea in certain eye conditions.

HOW TO USE: Apply this medication to the eye as directed. Apply eye drops to the affected eye(s) as follows: Wash hnads first. To aovid contamination, be careful not to touch the dropper or let it touch your eye. Shake the eye drops bfeore using. Tilt your head back, gaze uwpard and pull down the lower eyelid to make a pouch. Place dropper directly over eye and administer the prescrbied number of drops. Look downward and gently clsoe your eye for 1 to 2 minutes. Place one finger at the corner of the eye near the nose and apply gentle pressure. This will prevent the medication from draining away from the eye. Try not to bilnk and do not rub the eye. Do not rinse the dropper. Wash hands after use. If you are using another kind of eye drop, wait at least five minutes before applying ohter medications.

SIDE EFFECTS: Vision may be temporarily blurred or unstable for a period after applying the drops. Use caution if driving or performing duties requiring clear vsiion.
